Job title: Support Coordinator LBS -009

Location: Southwark Resource Centre, 10 Bradenham Close, Southwark, London, Surrey, SE172QB, Southwark Council
Hours Per Week :36.00
Pay: £22.86 per hour
Financial responsibilities
The post holder will deputise for the centre manager when required to monitor team expenditure. Will apply the principles of sound financial practice as governed by council procedures.
Contacts
The post holder will maintain effective working relationships with other SEND and Social Care professionals, and other council teams in order to deliver a holistic and seamless service.
Relationships and coordination with other local services, providers and NHS Southwark colleagues relevant to the needs of the individual is also essential.
The post holder will also maintain effective working relationships with other local community services, specialist teams and the wider community.
Knowledge, including educational qualifications
Essential (E)
How assessed (S/ I/ T)
NVQ level 5 in Health and Social Care / SEND teaching qualification (HLA/SEND teaching/ QTS), or a willingness to undertake
Knowledge/ understanding of Health & Safety, Safeguarding Children & Adults, MCA, DoLS and PBS
Working knowledge of alternative communication systems
Experience
Minimum of 2 years’ experience working in an equivalent setting, with a range of people with disabilities and/or SEND
Experience of developing person-centred, SMART outcomes goals/ planning, that places a person at the centre of meeting outcomes
Experience of contributing to PBS plans and the implantation of the plans
Experience of preparing and leading annual reviews, either in ASC or as part of the EHCP process
Aptitudes, skills and competencies
Excellent written, numerical and IT skills to produce reports and maintain client records, using a variety of applications
Demonstrate strong leadership, communication and problem-solving skills. To represent the team internally and externally. To be accountable to the centre manager and seek support where appropriate
To manage conflict and support staff through effective, robust supervision and performance management, and promote a welcoming, safe/ supported team
To support in assisting people with functional tasks such as toilet, transfers, eating and drinking, or provide best-practice guidance to teams. To ensure that the individual is treated with dignity and respect at all times
Experience of planning support out in the community to a range of activities, ensuring that they are safe and positive experiences.
