Job description
The Long-Term Adult Social Care Occupational Therapy Team provides assessment, advice and intervention to adults with long-term disabilities, health conditions, frailty, or age-related needs who experience difficulties managing daily activities and maintaining independence within their home and community.
The team works with adults aged 18 and over who have eligible social care needs, undertaking strengths-based occupational therapy assessments to identify functional difficulties, risks and barriers to independence. Interventions focus on enabling individuals to maximise their independence, safety and wellbeing through a range of approaches including:
• Assessment of daily living skills, mobility, transfers and personal care needs.
• Provision of advice, information and self-management strategies.
• Recommendation and provision of equipment ranging from simple/complex/custom made.
• Assessment for minor and major adaptations, including Disabled Facilities Grant (DFG) funded works where appropriate.
• Designated Duty day once per week triaging incoming referrals.
• Manual handling assessments and advice for informal carers and care providers.
• Moving and handling assessments for adults with complex physical needs.
• Collaborative working with health professionals, housing providers, care agencies and voluntary sector organisations.
The team adopts a person-centred and outcome-focused approach, supporting individuals to achieve goals that are meaningful to them while promoting independence, reducing risks and delaying or preventing the need for ongoing care and support services.
Occupational therapists operate within the framework of the Care Act 2014, relevant legislation, professional standards and local authority policies to ensure assessments and interventions are proportionate, lawful and responsive to individual circumstances.
