3 days in office, 2 days working from home – but flexible
JOB PURPOSE

To carry out social work tasks related to assessments, care planning and the development of support packages in order to promote the welfare of children. The post will have case holding responsibilities and also be responsible for developing support networks to Children with Disabilities their families and carers.

PRINCIPAL RESPONSIBILITIES

• To undertake initial, core and integrated assessments in compliance with the Assessment Framework
• To develop care plans and support packages
• To undertake statutory duties in respect of children looked after
• To undertake duties in respect of children on the Child Protection Register
• To undertake duties in respect of other Children In Need
• To input data onto Care First
• To undertake any other tasks as required
• To work within the framework of All Wales Child Protection procedures and Pembrokeshire Child Care Procedures
• To liaise with colleagues in education (including specialist residential schools) to ensure that provision meets the needs of the children and families.
• To liaise with colleagues in health to ensure that the specific health needs of children are identified and met within the assessment and care planning process.
• To liaise with colleagues in both Children�s social Care and Adult Social Care
to ensure that safeguarding, care and transition needs are met.
• To undertake such other duties of a similar nature and responsibility level as may be required from time to time.
• To undertake the role of practice assessor and/or practice facilitator for student social workers on placement.
PERSON SPECIFICATION

• Social Work Qualification and Registered with the Care Council.
• Practical knowledge of the Children Act (1989), the Social Services and Wellbeing Act (2014) and related legislation.
• Qualification/Training and/or experience with working with people with a physical and/or sensory disability desirable.
• A thorough working knowledge of the Microsoft suite of software.
• When 2 years post qualified, undertake to qualify and take the role of practice assessor and/or practice facilitator for social work students on placement
• Clear/concise written and verbal communication skills.
• Good organisational skills
• Valid driving licence and car available for work.
• Ability to work positively with other agencies.
