Principal Responsibilities

1. To complete assessment for Powys residents in district general hospitals to determine their discharge pathway and care needs for a safe and timely discharge from hospital.

2. To undertake timely strengths based proportionate assessments and care and support planning to assist Powys patients to move into the optimal Discharge to Recovery and Assess (D2RA) pathway following hospital treatment.

3. To liaise with other out of county acute/community hospitals to support the transfer of Powys patients from hospital back into their communities.

4. To support Powys residents on the continuum of hospital transfer through to identification of ongoing care needs following a suitable period of recovery.

5. To ensure that risks to Powys residents and other are assessed and balanced in a way that promotes safe transfers from hospital and a focus on independence.

6. To work in a multi-agency manner to help achieve what matters to the Powys residents being supported. This will include working in partnership to ensure, where appropriate, that primary health needs have been identified before Adult Services assumes responsibility for the organising of appropriate ongoing care and support.

7. To be part of and actively contribute to timely MDT/hospital transfer planning meetings.

8. To be accountable for practice through timely, effective and accurate recording of decisions in line with policy and legislation.

9. To address any capacity issues faced by Powys residents through leading on or contributing to the timely completion of Mental Capacity Act assessments and/or best interest decisions.

10. To support the Hospital Team in contributing to wider partnership initiatives, in the recognition that meeting needs and preventing hospital admission requires a whole system approach.

Experience

• Post qualifying experience of working with Older People/younger adults and their carers

• Experience of undertaking assessments of need and identifying and managing risks.

• Experience of complex casework

• Experience of care management across a range of health and social care settings

Knowledge

• Understanding of the needs of vulnerable adults and their carers

• Understanding of safeguarding and the issues affecting vulnerable adults and their carers

• Knowledge of current legislation, national guidance and best practice relevant to the post.

• Knowledge and implementation of the Mental Capacity Act

Qualifications & Training

• Appropriate social work, health or allied health practitioner qualification (i.e. Degree in Social Work, DipSW or equivalent).

• Registration as Social Worker with Social Care Wales/CQC

Safeguarding Requirements

• Requirement to undertake an Enhanced DBS Check
