Contact Cares is a large and busy multi-disciplinary team that is ever developing and currently consists of both qualified and unqualified Social Care, Nursing and Therapy staff . The team is based in Nightingale House within the grounds of Whiston Hospital. The team provide initial contact, assessment, care planning, onward referral and signposting services for a wide range of adult clients and on behalf of staff from across health and social care. With a focus that stretches from prevention and early intervention to reducing hospital admissions and readmissions the work is varied and interesting. The Urgent Community Response Service runs alongside the service with Health and Social Care staff working closely together. The roles will be responsible for Hospital discharge planning, screening cases and assessments for the Urgent Community Response Service and as part of more general Multi Disciplinary Team. It is primarily office based with outreach work to the Community Hospital and service user homes. There will be support and supervision and the opportunity to develop skills.
