Pay rates are negotiable depending on skills and experience
You will support and meet the needs of the service users and families to promote independence to disabled children and young people.

Role details:
*You will be responsible for carrying out assessments for children and young people aged 0-18 years who have severe and complex disabilities. The assessment is to determine the degree of their disability and to identify the most suitable method to support in overcoming or reducing the effect through the provision of specialist, equipment and adaptation of the home.

*You must be vigilant to safeguarding issues and must report any concerns about the safety or welfare of a service user.

*You will be required to advise and support children or young people and their family about the Disabled Facilities Grants system and alternative means of financing adaptations. You will need to encourage and assist the family to explore the pros and cons of an adaptation. And you will work closely with the architects to check the plans and specifications to ensure the design meets the child’s current and predicted needs.
*You will be expected to complete Housing Needs reports and viewing potential properties when appropriate.
*The role will require you to carry out and write up Risk Assessments, Handling Assessments and Handling Plans as appropriate.
*You will need to prioritise and manage a busy caseload with complex cases.

You will be required to travel widely within the Authority so you must either have a current driving licence and provide a car, or have access to appropriate means of travel.

Essential
Degree in Occupational Therapy or equivalent. (KSE1)
Registration with the Health Care Professionals Council (HCPC). (KSE1)
Experience of working with a broad range of people including, disabled people, families, partners in a social care setting and providing solutions including, reablement, rehabilitation, Telecare, equipment and adaptations. (KSE2,5)
Demonstrate good understanding of disability on activities of daily living (KSE2)
Experience of, or be prepared to work in accordance with the Councils Safeguarding policies and procedures (KSE3)
Evidence of on-going continuous professional development and knowledge of current legislation and practice relating to social care. KSE1,3)
IT skills to use client data system (with training if needed), and common software packages e.g., word, outlook (KSE7)
Have good interpersonal skills, be able to carry out good quality assessments and outcome focused plans and communicate findings and risks to service users, families, carers and keep detailed and accurate records. (KSE3,5)
Full clean drivers licence and assess to a car (or able to use another appropriate means of travel) to travel to a range of locations including home visits.
Desirable
Considerable post qualifying experience (KSE2)
Some skills or experience in supervising and mentoring others (KSE4)
Qualified Best Interest Assessor (KSE1)
Experience of working with complex cases (KSE2)
Experience of working across other organisations i.e., health, third sector (KSE2)
