The purpose of the All-Age Complex Placements Officer is to be responsible for securing placements for service users with complex needs, arranging individual placement contracts, ensuring value for money and positive outcomes for individuals.
Main Purpose of the Job/Key Objectives:
To commission care packages for complex cases specialising in people with Learning Disabilities, Physical & Sensory Disability, Dementia or Mental Health issues
To commission, where appropriate, approved packages of care with Reablement and homecare providers on the Domiciliary Care Framework Contract, and ensure timely, safe and smooth transfer of those clients in the care of those providers.
To commission Respite and Long Term Residential/Nursing Home Placements for clients and ensure timely, safe and smooth transfer of those clients to their chosen Home.
To build market intelligence and document it so that there is an understanding of reasonable rates and quality of provision for others to access as well as building the knowledge of the post holder
To work closely with the Placements team to ensure placements are recorded on all appropriate systems including establishment and monitoring of finance records aligned with appropriate IT systems
To work closely with the Quality service where issues arise in relation to the quality of provision
To build/ maintain a tangible and accessible record of services, people placed in them with data like date of placements, reviews and costs.
To support the identification of placements where outcomes are not being progressed and to work closely with operational colleagues to challenge provision that is not as supportive or independence building as it could be
To be aware of voids in locations and to link them to appropriate placements so that capacity is used as effectively as possible
To take a project management approach to improvement opportunities that are identified to improve the system in regard to complex cases and how they are managed
Experience:
Of working in a brokerage/ commissioning function.
Of dealing with external providers for the provision of care services.
Of working within Social Care
Of dealing sensitively with issues that can reach across many stakeholders, including providers, families and operational services
Qualifications
Graduate qualification or equivalent experience.
Project management qualification or equivalent experience.
Working conditions/circumstances
Office based
Be able to deputise for Line Manager at service/other meetings as required.
