Exciting opportunities have arisen for qualified Social Workers to join our award-winning Integrated Community Services (ICS) Team.
The service encourages a ‘Home First’ approach, ‘right care, right time, right place’, working closely with our in-house CQC Registered reablement service, START and partner agencies.
As a Social worker you will be actively involved in facilitating complex discharges from Acute and Community Hospitals within Shropshire and within neighbouring counties to repatriate Shropshire residents back home whilst also providing an Admission Avoidance service.

The job entails undertaking a wide range of duties, so a flexible approach to your work is required. The role includes working under pressure at times of high demands from our system partners and having personal drive.
In collaboration with other professionals make assessment of individual need and implement and monitor the agreed support plan and keeping safe plan, as appropriate.
Consider and assess Mental Capacity and co-ordinate Best Interest decision making forums where appropriate. Documenting decisions and assessments accurately.
The ability to recognise where a Deprivation of Liberty or a restriction may be an issue. Liaising with registered providers when a referral to the DOL’s Team is appropriate.
To contribute to the development of all aspects of the team and the service provided.
To co-ordinate the use of all resources promoting the creative and flexible use of personal budgets to meet eligible needs and accessing community services and activities
To maintain accurate records with relevant, objective, detailed information and with accuracy in a timely manner and in accordance with departmental policies and procedures.
To attend and participate in team meetings, supervision and performance and appraisal and any quality assurance processes.
To take part in duty systems, as required.
To comply with safe systems of work in accordance with the Department’s Health and Safety and Welfare policies and procedures.
To develop an understanding and awareness of issues related to equal opportunities, valuing people and anti-discriminatory practice and to demonstrate this in carrying out your duties.
Responsible for promoting continuing personal and professional/vocational development of self through staff development processes.
We welcome applications from experienced and newly qualified Social Workers. Shropshire has a strong professional development culture and regular supervision is provided. Successful candidates will be aligned to one of the five area teams – Central, North East, North West, South East, South West – but will be required to work flexibly across the county as and when service demand requires.
The post holder will be expected to adopt a flexible attitude to the duties of the post. These may be varied after discussion between the post holder and management team to take account of the changing needs and development of the Service areas.
