This role requires you to work as an Adult Social Care representative within the Continuing Healthcare assessment process and contribute towards the decision making activity which specifically relates to an individual’s CHC eligibility and where appropriate to do so, complete Social Care assessments in preparation of attending MDT joint assessment meetings. This will include being directly responsible to progress identified cases through Adult Social Care and the CCG’s jointly agreed dispute process.
The assessment activity related to the above post is Countywide and will require you to attend planned Multi-disciplinary meetings throughout Shropshire. This will also include the occasional need to contribute to health lead out of county meetings where the individual to which the meeting relates to is a resident of Shropshire.
Main Duties
⦁ Providing an excellent service for people identified as needing social work support;
⦁ Attending and contributing to the CCG’s Continuing Healthcare assessment process
⦁ Develop positive relationships with regard to the joint responsibilities relating to the CHC assessment duties.
⦁ Providing appropriate challenge where there is clear and evidenced information which may be used within a recognised dispute process.
⦁ Liaising with other professionals as required;
⦁ Managing workload, receiving referrals, responding to the needs and requests of the person as appropriate.
⦁ Working in a timely and efficient manner.
⦁ Focusing on outcomes and promoting the person’s independence, choice and control;
⦁ Focusing on the person’s strengths, natural and community support networks in achieving outcomes;
⦁ Identifying alternative ways of achieving outcomes as appropriate (e.g assistive technology etc).
⦁ Working effectively as part of a team and having a positive and solutions- focused approach
⦁ Following agreed policies and procedures
⦁ Working closely with the person, their family, other agencies and the police to safeguard vulnerable adults from all forms of abuse and exploitation and will adhere to the multi-agency Safeguarding of Vulnerable Adults Policy.
⦁ Support and attendance at the NHS Independent Review Panel (IRP) regionally in negotiation and agreement with line manager to support the process and contribute to decision making.
