Mental Health Social Worker.
Duration- up to 3 months with review for potential extension.
Immediately, working remotely / office / multiple sites.

This role will support existing vacancies in the team which are due to internal secondments. These vacancies are urgently needing to be filled due to winter pressures.

Context:
The Mental Health Social Care Team is Currently based at Tolworth Hospital.
ASC has a hybrid model of working and MHSCT has their own charter Team Manager & Deputy Manager for MHSCT, DOLS, AMHP Services 11 Social Workers (excl. team manager and deputy team manager); plus 3 F-T AMHPs, 1 support worker, and admin.
Google Chromebook, work mobile, and adaption for office / work from home.
Mental Health Social Work / Care Management.
Working with Functional Mental Health problems, Autism, Transitions, Drug and Alcohol problems.

Will undertake:
• Duty cover for MH Social Care Team
• Shadow duty cover for MH social care team
•  Care Act Assessments
• Eligibility Decisions, signposting
• Care Planning

Commissioning:
• Specialised outreach support
• Homecare
• DPs
• Supported Living, Residential and Nursing
• Working with Supported Living and Homecare Frameworks
• SAM and / or Enquiry Officer in Safeguarding Investigations
• Professional Social Work Support

Responsibilities:
• Attend Accommodation Panel to present Care Act Assessments
• Active participation in Team meeting, allocations, SGA meetings
• Joint Review supported living and residential care placement with health representatives from the Mental Health Trust and GP.
• Promote step down in supported living, recovery & independence
• Crisis Intervention and social work support for those living independently incl. appointeeship.
