This social work position is for a specific project based in the Adult Community Rehabiltation Team. This is an interdisciplinary community rehabilitation team with two specialist pathways for clients living with physical or neurological long-term conditions. The team is made up of Physiotherapists, Occupational Therapists, Speech & Language Therapists, Clinical Psychologists, a Neuropsychologist, Rehabilitation Assistants and Administrators. The team is supported by a Consultant Geriatrician, Consultant Neurologist and an AHP Consultant.

This post is predominantly community-based and the post holder would be required to have a flexible approach to working across different sites between Hackney Service Centre and St Leonards.

This role would be responsible for working collaboratively and proactively with ACRT clinicians, using a strengths-based approach to manage the complex needs, facilitate self-management and support independence of clients living with long-term health conditions. The postholder would be required to attend multi-disciplinary meetings, undertake assessments, advice and promote personalised support to achieve outcomes of focused support plans. The postholder would also identify, support and facilitate training opportunities across both partners and facilitate a shared approach to accessing partner information.
