Salary: £20.48
Rate of pay is £32 per hour-dependent on experience.
HCPC Registration
Social Worker required for the Integrated Discharge Service.

Requirements:
• Clear experience and operational knowledge of working in a busy and pressurised front-line team.
• To work to achieve zero delays and admissions avoidance.
• Have a clear understanding of and experience of working with Adult Social Care and its statutory duties under the Care Act 2014, Mental Health Act, Mental Capacity
• Act and other primary legislation and guidance including Continuing Health Care.
• To complete face to face visits. To demonstrate person centred working and to facilitate needs based assessments using a strengths-based approach.
• To be able to work under pressure, meet tight deadlines and work as part of a multidisciplinary team.
• To be able to record to a high standard and to demonstrate evidence based work.
• Must have experience and knowledge of safeguarding and section 42 enquiries.
• Work in partnership, formulate and implement personal budgets and support plans.
• Assist and participate in the management of the duty service and carry out screening of clients to determine criteria to be supported by the service.
• To be expected to also work to the D2A (discharge to assess) pathway and complete community care act assessments.
• Home working will be available.

Requirements of the Social Worker:
• Degree or equivalent (diploma/CQSW) in social work.
• Social Work England registration.
• 1 years previous experience as a social worker within adult services, particularly hospital discharge social work.
