3 days a week (0.6FTE) in CLA Team – to hold a small caseload of settled young people who are placed out of Bracknell’s LA area. The aim of the role is to have a SW based out of Bracknell’s LA area who is closer to the young people and therefore reduce the travelling pressure of the permanent team members and can provide additional support for the young people for quality of service and care.
You will be responsible for case-holding these young people; completing all visits and tasks for their care planning/ensuring their needs are met.
Reduced requirement to attend the office as per the reasons for this role – monthly for team meeting and face-to-face supervision. You will not be required to cover Duty days.
Caseload will be 11/12 young people – settled meaning they have been in their placement for sometime and the majority are long-term matched. Young people currently placed in and around; Canvey Island, Essex, Croydon, Westgate-on-Sea, Herne Bay, Hollingbourne, Margate. Therefore looking for someone based East London/South Kent/Essex – easy access to A2 and surrounding area.
