We have an exciting opportunity for a hospital senior social worker post, based at Frimley Park Hospital, full time
This will be up until end of March 2023, to support and guide any Bracknell residents out of Frimley Park Hospital A&E and AMU. The posts will be based at Frimley Park Hospital Monday to Friday, covering core business hours.

Due to continued pressures on the hospitals, people are being discharged home once medically optimised. There is extreme pressure on acute beds and we want to work with the trust in supporting a timely and smooth discharge. There is a requirement to be based within the Discharge team at Frimley Park Hospital but you will be line managed by the Hospital team based in the Adult Community Team in Bracknell.
