We are looking for a passionate, motivated and experienced Social Worker to join our Mental Health Team. This is a small, well-established, dedicated social care service for people with severe and enduring mental health conditions.

The mental health social work teams provide specialist services for adults over 18. The teams support people to recover from periods of difficulty with their mental health and for people to stay as independent as possible by providing support that is person-centred which gives people choice and control in their lives.

As a Mental Health Social Worker, you will also be involved in assessing, coordinating, and arranging care for service users with mental health problems, including dementia, both in the community and in the hospital.

You will need to be Registered with Social Work England / your appropriate Professional body. Experience of working in a health or social care setting is essential, along with excellent communication skills. You will need to demonstrate passion and enthusiasm for integrated working and an understanding of how we can best engage with our local communities and other partners in a meaningful way.

Working model: Hybrid (50% of working hours from home)

qualifications
Degree or equivalent experience/skills.
ITQ 2 or equivalent skill and ability demonstrating significant experience in IT systems.
Professional qualification in area of specialism.
Licence / certificate / qualification required for the role.

Some relevant professional experience post qualification in a similar work environment.
Expert knowledge of relevant policy, systems, work practices, professional guidelines, legislation in
the area of specialism.
Excellent ICT skills including use of Microsoft applications and specialist systems.
Experience of multi-disciplinary and partnership working and awareness of the issues involved.
Planning and workload management skills.
Proven assessment and investigation skills appropriate to the scope of the role. Ability to carry out
complex client assessments.
Experience of using professional knowledge to work within referral systems according to
established criteria.

Accurate record keeping and report writing.
