Salary: £22.95
We require a qualified social worker experience in mental health to review supported living placements. The majority of these reviews with be undertaken jointly with Health partners. There are clear objectives with a view to facilitating step down in order to maximise independence, and enable better outcomes.

The post holder will need demonstrable experience of working in a health or social care management role, and experience of housing and supported living facilities with relevance to the needs of people with severe mental health problems.

The following will be essential:

Experience of assessing the needs of people with severe mental health problems for varying types of accommodation and step down.
Experience of working with people who experience serious ongoing mental health problems
Experience of working in a multidisciplinary team
Experience of working independently to set objectives.

Specialist knowledge of effective models of mental health and social care provision
Experience of Reablement/ Recovery / Strengths based working.
Knowledge of Safeguarding issues and how these apply to the role
Knowledge of social care legislation
Clear understanding of the Mental Health Act, and National Policy, MCA and DOLS
Skills in assessment, care planning and case management
Ability to build relationships across organisational and professional boundaries
Excellent verbal and written communication skills
Good time management skills and ability to prioritise
Good influencing skills
Intermediate IT skills
Ability to demonstrate familiarity with information technology in order to produce reports and statistics
Report writing skills
Ability to work under pressure
Ability to work to ensure effective communication with relevant partners both within and outside the services

TRAINING & QUALIFICATIONS ● Educated to degree level in a health or social care discipline
● Evidence of continuing professional and personal development

EXPERIENCE ● Demonstrable experience of working in a health or social care management role
● Experience of housing and supported living facilities with relevance to the needs of people with severe mental health problems.
